Welcome! We are very excited to have your scout and family join our Pack. To be part of our pack, you must complete an application/membership packet. The packet consist of the following forms and electronic versions are attached below.
Also included below is an "Explanation of Den Levels & Scouting Year", a "Membership Checklist" for Tiger and Cub Scouts (Wolf, Bear & Webelos), and Leaders, as well as "Membership Information".
SCOUT PACKET
- The electronic version can only be printed and completed in writing.
- Parent must sign the form in the lower right-hand corner.
Annual Health & Medical Record
- Can be completed electronically on the form. We recommend saving a copy of this form on your computer to easily and quickly make updates.
- Must include immunization records or a copy of your scout's immunization card.
- Requires a copy of your insurance card - front & back.
- On the paper form or printed electronic version, parents must sign the last page.
Activity Consent Form
Parents must complete for their scout to participate in Pack 3062 Den Meeting, Pack Meeting and activities (at Pack location and off-site locations.)

Below is information regarding the uniform requirements as well as the address and location of the scout store.
Tiger Parent Training
Tiger Parents are required to watch the Youth Protection Training online training by creating an account at www.myscouting.org and click on create new account. From there click on new to scouting click here to take training. This is a required training for all new parents. It takes approximately 30 minutes. Please print out your certificate and take note of your BSA member number.
PACK LEADERS & TIGER PARENT PACKETS
Adults who are Tiger Parents or volunteering in a leadership position - Den Leader, Assistant Den Leader, or Committee Member - need to complete an application/membership packet.
- On the paper form or your printed electronic version:
- Adults must sign the backside of the form under "My Signature indicates".
- Adults must sign the application page in the lower left-hand corner.

Parent Training - Youth Protection Training Certificate
Tiger Parents and Leaders are required to watch the
Youth Protection Training online training by creating an account at
www.myscouting.org and click on E-Learning to take the course. This is a required training by the Boy Scouts of America. The training is approximately 30 minutes. The training certificate is required to be submitted at the time of application.
